Introduction

The chosen assessment topic for this group task is Documentation in Nursing Health Practice which aims at preparing a comprehensive documentation teaching plan to teach the peers about the importance and specifics of how to collect and record patient’s vitals in an accurate manner. Proper documentation of the patient’s vitals helps in the maintenance of continuity of the caregiving process, reduces duplication errors in patients' care as well as saves time. There exists a significant knowledge gap in the aspect of documentation which is one of the main topics of learning in clinical healthcare. Since poor knowledge in this field ensures difficulties for both the nurses and patients, it is important to develop this teaching plan for the peers through useful teaching methods to promote their skills and knowledge in this context (Ebrahimpour & Pelarak, 2016).

Nursing learning theories are crucial in gaining holistic knowledge of the process of healthcare, of which documentation is a part. Nursing learning theories are based on three concepts, behaviouralism, cognition, and constructivism. Behaviouralism deals with the observable side of learning in which nurses can understand their patient’s condition based on what they observe about them. Next comes cognitive abilities which include making assumptions based on behaviors and involves prior knowledge and experience as it depends upon behavioral indicators to learn about the patient’s condition. Thirdly, constructivism includes the postulation of new theories and notions based on their knowledge and past experience and prepares them to face challenges in real-time (Mann & MacLeod, 2015).
